I need a deep pathophysiology knows why increase the level of hematocrit in patients who smoke.

This is a profound answer:
Smoking can cause this in a few ways.
First, with biochemical changes which may lead to mild dehydration associated. This can cause a relative increase of the component cells (hematocrit) compared to plasma (water) components, thus increasing the% of hematocrit.
Secondly, toxic smoke red blood cells (GR) with carbon monoxide, so that the cells are not capable of binding oxygen. this sense organ and creates more red blood cells (hematocrit, in other words) to get more oxygen (which of course toxic, but that's another story).
